1. How do I register my child for school? How
do I register my child for school after the start of the school
year?
To register your child for school, you pick
up a registration packet from your neighborhood school. If you have all the necessary documents, you turn the packet into the school. If you do not have documents, you bring it the the District office.

2. How do we apply for an inter-district
transfer?
If you would like to transfer into the
San Leandro district, first go to your local district office
to obtain the proper forms. The inter-district transfer process
begins at your home district.

3. What immunizations are required
to enroll my student?
Kindergarten: children age 6 and under
(or First Grade, if first entry to school)
- Polio: 4 doses (3
doses OK if last dose was after 4th birthday)
- DPT, DTaP: 5
doses (4 doses OK if last dose was after 4th birthday)
- MMR:**
2 doses (Both doses on or after 1st birthday)
- Hepatitis B (HBV):***
3 doses
- Varicella (Chickenpox):** 1 dose
(If child has had Chickenpox, it MUST be noted on immunization
record by the doctor)
Children ages 7 through 18
- Polio: 4 doses (3 doses OK if last
dose was after 2nd birthday)
- DPT, DTaP: 4 doses (3 doses OK
if last dose was after 2nd birthday)
- MMR:** 2 doses (On or after
1st birthday)
- Varicella (Chickenpox):** All new entries to California
schools
Ages 7-12: 1 dose
Ages 13-18: 2 doses
Seventh Grade Entry
- Hepatitis B (HBV):*** 3 doses
- MMR:*** 2 doses (Both doses
on or after 1st birthday)
** Sarampion
(given in Latin American countries) is measles only, not
MMR
*** HbOC on immunization record is NOT Hepatitis B
Students
who have not completed the above immunizations will not be
admitted to SLUSD schools. Students with an incomplete record
maybe be admitted if it is too soon for the next dose.

4. What health screenings are conducted
by the school district?
Vision screenings are conducted in
kindergarten, 2nd, 5th and 8th grade. Hearing screenings are
conducted in kindergarten, 2nd, 5th, 8th and 10th grade. Scoliosis
screenings are conducted for girls in the 7th grade and for
boys in the 8th grade.

5. What is an IEP?
An IEP is an Individualized Education Program.
Each public school child who receives a special education and
related services must have an IEP. Each IEP is designed for the
individual student and creates an opportunity for teachers, parents,
school administrators, related services personnel, and students
(when appropriate) to work together to improve educational results
for students with disabilities.

6. My child has a food allergy. What should I do?
If
a child has a food allergy a medical form needs to be filled
out and signed by your doctor. It is helpful if the Doctor suggests
replacement foods. The medical form needs to be filed in the
Food Services Department before any changes or substitutions
can be made to the menu. Forms are available from the Food Service
Office.

7. Why do I need to apply
every year for Free or Reduced Price Meals?
The
Federal Government requires families to complete an application
every year. The reason for this is that circumstances in families
change from year to year. As well as the income eligibility guidelines
set by the Federal Government change each year.

8. Do I need to fill out
an application for each child in my family?
Only one application
per family is required. All household members need to be listed
including children that do not attend school.

9. What meals are served
at school?
Every school
site offers breakfast and lunch. See school menus here.

10. Can
I prepay for meals so my child doesn’t need to carry money
to school?
Yes
students can prepay for meals. The computer system works like
a debit card and only subtracts money when the student eats.
Prepaying for meals takes the worry out of money lost or left
in the school desk.

11. What is the purpose and focus
of the Physical Education program in SLUSD?
The purpose and focus of the Physical
Education program is to develop and promote good health, advanced
motor skills, improved self-confidence, and increased levels
of physical fitness in all students through the implementation
of a high-quality, standards aligned program. The Physical
Education department is currently in the process of creating
a 5 year strategic plan to strengthen and support the delivery
of a high-quality Physical Education program in SLUSD.

12. I have heard that the school district has a
visual and performing arts plan. Is this true, and what does
it mean?
Yes. The purpose and focus of the SLUSD
Visual and Performing Arts plan is to have the San Leandro
community (teachers, administration, students, parents, and
community) understand the following:
- The breath and depth of a quality arts education program
- What is need to build and support a quality arts education
program that meets the needs of all students.
- The importance of providing on-going and varied funding avenues
to sustain arts education programming.
The San Leandro Visual and Performing Arts program is part of
the Model Arts Project Network through Alameda County Office
of Education Alliance for Arts Learning Leadership.

13. How
do I obtain a work permit?
San Leandro High School students can
obtain work permits during the school year at the school
site from the attendance office. During the summer vacation
student should come to the district office and visit Student
Support Services for the work permit. Lincoln High School
students should come the district office year round to obtain
a work permit.

15. Where do I get a school facilities rental application?
You can come to the District office or download
an application from our website. Once completed you
may submit the application to our Community Services Department.

16. Do I have to provide insurance coverage
for the use of the Facilities?
Organizations or Groups using our facilities
would have to provide a certificate of insurance from their insurance
carrier in the amount of $1,000,000. San Leandro Unified
School District would have to be named as an additional insured
under the organization or group liability policy.
